CES 95 Families, Updated March 21, 2024

 

Current PreK, 5th, and 8th grade families, your students have requirements for the upcoming year. These students are entering the grades in which new physicals are mandated and additional immunizations are required. Also, if you are new to the district or did not have something turned in from last year, please see below.

 

Reminder of what is needed:

 

  • Kindergarten - Physical and up to date immunizations given at this grade level/age, an eye exam completed by an optometrist, and a dental exam.
  • 2nd grade - Dental exam
  • 6th grade - Physical exam with 6th grade booster immunizations (Tdap and Meningococcal) and a dental exam. 
  • 9th grade - Physical exam and a dental exam.

 

We are partnered with Shawnee Health to increase the access to healthcare for our students and their families. Terrier Care & Shawnee Health are both able to complete physicals to count for next year's grade level requirements. Schedule your physical exam/immunizations through Shawnee/Terrier Care at (618) 519-9200 or (618) 956-9690. If you need assistance with setting these up, please reach out.

 

If your child is already established with a provider, please make an appointment with them as soon as possible to schedule IL physical exams and immunizations (if needed for your child).

Upcoming Health Events:
  • Felicia Kimbrough, APN, DNP from Terrier Care, will be coming to Lewis School to provide physicals and immunizations to fulfill the 6th grade entry health requirements.
  • Dates to participate are: April 15th & 22nd
  • Please contact her office at (618) 956-9690 to schedule an appointment. Registration and consents MUST be completed prior to scheduling.

 

How to Participate:

 

  • Registration packets went home to all 5th grade students on 3/20/24.
  • IF you would like your child to be seen at our school, please complete paperwork and turn in as soon as possible.
  • Ms. Felicia will be calling the parent/guardian before and after the visit.
  • You can video in during the visit if you are not able to attend but would like to be present.

 

If your child is already established with a provider, please make an appointment with them as soon as possible to schedule a 6th grade physical exam and immunizations (if needed for your child). A copy of your child’s immunization/physical will be coming home. Please complete the highlighted parent portion and take that into your providers office.

 

Students Participating in Interscholastic Sports:

 

  • Students wanting to try out for any interscholastic sport(s) are required to have a physical examination performed yearly and submitted prior to be eligible to "tryout." 
  • The 6th grade - IL Physical form meets the need for qualifications for interscholastic sports.

Additional Health Information:

 

*Parents, it is imperative that you inform the nurses as well as the student's school if your child has any health concerns such as asthma, allergies, and or any other health complications/disorders that may disrupt their learning. This should also be reflected in the registration form (health information section).
 

Parents of students diagnosed with asthma. The school will need an emergency asthma action plan that has been written by the student's provider or prescribing provider as well as providing the inhaler to be self-carried or placed in a central location for use at school. 

 

**If students with asthma are symptomatic and have no inhaler at school, the school will have to contact emergency services (911) for support in accordance with district policy.

 

Likewise, if your student has diabetes or any other health related needs requiring resources such as a glucometer, testing strips, lancets, etc. you are responsible for supplying all supplies.

 

If a student has an emergency action plan on file for health related needs (diabetes, asthmas, seizures, etc.) we must have the rescue meds available/provided prior to the start of school or the students first date of attendance.
